Transitional Care Support Specialist

Position Overview
This role presents a unique opportunity to engage with the Sutton Virtual Ward, integrating clinical responsibilities across both hospital and community settings while overseeing a team of Urgent Response Nurses.
Collaborating with multidisciplinary teams, you will evaluate patients in their residences who have been discharged prematurely from medical facilities or to prevent hospital admissions when feasible. Additionally, you will operate within the hospital environment, identifying and assessing appropriate patients, fostering relationships, and educating fellow staff members.
Your contributions will be vital in establishing trust and confidence in the Virtual Ward initiative and in enhancing both new and existing clinical pathways.
Key Responsibilities
- Conduct comprehensive holistic assessments of patients in both community and hospital settings.
- Manage a team of Band 6 nurses, including responsibilities for sickness management, performance evaluations, and professional development.
- Assist the operational manager with scheduling, leave management, and other administrative tasks.
- Lead audits and compile reports as directed by the operational manager.
- Participate actively in multidisciplinary team meetings and contribute to decision-making for patients under the Virtual Ward's care.
- Collaborate closely with the Virtual Ward team to ensure efficient daily operations.
- Take charge of your own professional growth and development.
- Balance clinical and administrative duties, effectively prioritizing and managing your time.
- Serve as an enthusiastic and knowledgeable advocate for the Virtual Ward.

Education and Qualifications
Essential Criteria
- Specialist Practitioner qualification or Advanced Practitioner at degree/masters level or equivalent.
- Evidence of ongoing professional development.
- Prescribing qualifications as part of specialist practitioner training.
- Proficient in Physical Assessment and Clinical Reasoning skills.
Desirable Criteria
- Qualification as a Specialist Practice Teacher.
Experience
Essential Criteria
- Recent experience as a senior nurse in a community setting post-registration.
- Proven ability to manage the needs of complex patients in a community context.
- Experience in supervising, mentoring, and evaluating staff performance.
- Assessment and supervision of pre-registration students.
- Demonstrated ability to work independently.
