About the Role

We are seeking a skilled and dedicated Nurse to join our team at Epsom and St Helier University Hospitals NHS Trust as a Community Care Specialist. This is an exciting opportunity to work in a dynamic and innovative partnership that provides integrated services for the local community.

Key Responsibilities
  • Work as part of a multidisciplinary team to provide high-quality care to patients in their homes and community settings.
  • Contribute to the development and implementation of care plans that meet the individual needs of patients.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
  • Provide education and support to patients and their families to promote health and well-being.
  • Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
Requirements
  • RN (adult) with NMC Registration
  • Relevant post-registration study at degree level and relevant Continuing Professional Development (CPD)
  • Mentorship course or equivalent
  • Advanced physical assessment or working towards
  • Independent prescriber or working towards
  • Extended Nursing Skills or working towards
  • Experience of conflict management
  • Experience in managing the needs of complex patients in relevant care settings, community or unscheduled care
  • Care Planning
  • Experience in supervising, mentoring and appraising staff
  • Holistic Assessment skills
  • Effective clinical skills
  • IT skills including keyboard and internet skills
