About the Role

We are seeking an experienced Registered Nurse to join our Sutton Health and Care team as a Community Health Specialist. This is an exciting opportunity to work in a dynamic and innovative partnership of providers, delivering integrated services for the local community.

Job Responsibilities

The successful candidate will be responsible for providing high-quality nursing care to patients in their homes, working closely with the multidisciplinary team to prevent hospital admissions, accelerate discharge, and provide supported discharge through the discharge to assess pathway.

Key responsibilities will include:

  • Conducting holistic assessments and developing individualized care plans
  • Providing expert nursing care and support to patients and their families
  • Collaborating with the MDT to ensure seamless care transitions and effective communication
  • Participating in quality improvement initiatives and contributing to the development of best practices
Requirements

To be successful in this role, you will need:

  • Current registration with the NMC as a Registered Nurse (Adult)
  • Relevant post-registration study at degree level and continuing professional development
  • Mentorship course or equivalent
  • Experience of conflict management and working with complex patients in a community or unscheduled care setting
