Job Responsibilities
  • Provide holistic assessments and care planning for patients with complex needs
  • Work collaboratively with the multidisciplinary team to ensure seamless care transitions and effective discharge planning
  • Develop and maintain strong relationships with patients, families, and carers to ensure person-centered care
  • Contribute to the development and implementation of care pathways and protocols to improve patient outcomes
Requirements
  • Registered Nurse (Adult) with NMC registration
  • Relevant post-registration study at degree level and relevant Continuing Professional Development (CPD)
  • Mentorship course or equivalent
  • Experience of conflict management and managing complex patient needs
