The post holder will work as part of the Community Response Team and provide Urgent Care Response to manage patients in the community setting, avoiding A&E attendance and hospital admissions. The post holder will support the clinical team in clinical triage and allocation of referrals.

The CRT works in collaboration with North West Ambulance Service (NWAS), Primary Care, Care Homes and the wider healthcare economy, with a focus on safe admission avoidance and supporting patients in crisis by managing their care and treatment at home. The team also have a key role with patients being discharged from hospital to ensure safe effective discharge.

Experience in the NHS is essential and Community experience is desirable.

This is a new post, currently going through job matching. It is expected to be evaluated as a Band 7.
- To manage the coordination of the team each day and supporting manager in expansion of the admissions avoidance service.
- To provide clinical leadership and support for the assessors and support workers.
- To support delegated activity from the Advanced Clinical Practitioners.
- The post holder will take a lead in the screening and allocation of all referrals into the admission avoidance team.
- To work as part of the integrated multi-disciplinary team and be responsible for assessment and management of a clinical caseload.
- To undertake comprehensive assessments of patients with complex presentations.
- To establish individualised management and treatment plans, using clinical reasoning and utilising a wide range of evidence-based treatment skills.

The role will require you to work across all areas in the Trust which includes those that are designated as positive, asymptomatic and negative COVID wards.

Based in the Community division so you will need to be required to travel across the borough.

On-Call

Please note that senior positions (AFC band 8A or above) may be expected to participate in an on-call rota, if the role is predominantly operational.

Key results from the job holder
- Delivery of Key Performance Indicators (KPIs) on care assessment, care delivery and workforce.

Planning and Organisational Duties
- Work within the Trust's Policies and Procedures and the parameters set down in agreed local clinical protocols and guidelines and national guidelines such as those published by NICE.
- Organise the clinical management of individual patients, utilising clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes where required. This will include monitoring and evaluating the responses to treatment through analysis and interpretation of clinical tests and investigations and the discharge of the patient.
- Facilitate the development of clinical practice through research and education of the multidisciplinary team, by integrating theory with practice.
- Evaluate care through quality assurance initiative and assessment of clinical outcomes.
- Be an expert clinical resource for other health care practitioners.
- Take every opportunity to promote the health of patients and initiate health education strategies as necessary.
- Participate in risk management and risk assessment working closely with other members of the clinical team.
- Continually develop own professional practice as indicated by the changing needs of services / patients
- Participate in the development of the quality and audit strategy, which includes standard setting and development of audit tools.
- Participate in developing and updating clinical protocols in care in responses to advances in clinical practice.

Communications and Key Working Relationships
- Accept, make and receive referrals to and from other health care professionals.
- Communicate patients' conditions, verbally and in writing, to other members of the healthcare team and contribute to decision-making processes in patient management.
- Communicate on an on-going basis with the GP, Community Medical Lead, Senior Registrar / Consultant responsible for the patient - updating him / her on changes/progress.
- Participate in virtual ward rounds, perform case presentation and contribute to the decision-making process in patient management.
- Act as a resource, providing clinical leadership and facilitating professional development amongst the multidisciplinary team both within and outside the organisatio
