Based on the Aintree Hospital site, you will work as part of the integrated multidisciplinary team to deliver reablement across 3x Orthopaedic wards you will work closely with nursing/therapies to support patients being prepared for discharge and stepped down into the community.

Reablement practitioners will work alongside clinicians assisting patients in their rehabilitation and recovery across orthopaedic wards.

The reablement role supports preventing deconditioning and promotes exercise, activity and recovery. These roles are based in the ward environment, and you will work within a small team as autonomous practitioners to support in the rehabilitation of all Orthopaedic patients. The patients on your caseload will have a variety of Orthopaedic issues that may include: fractured neck of femurs, wrist fractures, ankle fractures, Upper limb fractures, tendinopathies and spinal fractures.

There are many Skills that are required for the role including

moving and handling skills,

the ability to follow a treatment plan and

the ability to work well within the multi-disciplinary team.

Assisting people with activities of daily living and promoting safe mobility, with the use of various adaptive equipment.

Teamwork and effective communication

The ability to form effective relationships with both patients and their families/carer.

You will be supported by the therapy and nursing staff and regular supervision will be provided.

Your goal as a reablement practitioner will be to support the patient within their acute hospital stay to achieve the outcome of returning home and achieving maximum independence. We foresee this will be an extremely rewarding role.

We are an acute hospital based on the outskirts of Liverpool. These posts will be based at Aintree Hospital this is a major trauma centre for Cheshire, Merseyside and isle of man.
