An exciting opportunity has arisen for a confident, enthusiastic and motivated individual to work within the new Brent Health Matters Clinical Services at Central London Community Healthcare NHS Trust. The purpose of this service is to work directly with local communities in Brent, through GP’s referral’s, community leaders and voluntary organisations, to engage with people who have difficulty accessing health services, to improve health education, healthcare needs and some of the social determinants of health inequalities.

You will work within the Health Inequalities team alongside our senior nurses, AHP’s, GP’s and admin members, in supporting patient’s to regain and sustain their levels of function and self-management, with respect to their long term conditions.

The role will also require you to support associated Brent Health Matters partners through a variety of community engagement events. This role is patient facing, with an expectation to work collaboratively with carers, colleagues in health and social services, involve assessments and treatments for patient’s with a wide range of complex physical and social problems within a community setting.

The post holder is required to be flexible to assist where the needs are greatest.

The post holder MUST have a valid, FULL UK driving licence.

The Healthcare coordinator will work with the Health Inequalities team, alongside ICP, GP Network teams and other professionals to manage a caseload of patients around the management of long term health conditions and/or other issues that adversely affect patients’ health

To work as a healthcare coordinator in supporting patients to regain and sustain their levels of function and self-management

To be involved in patient care, defining treatment needs in relation to their healthcare needs and carrying out this treatment, maintaining clinical records.

Undertake the learning of new skills and knowledge to include some nursing, physiotherapy and occupational therapy skills. All these skills are competency led.

**Weekend and Evening working will be required as part of the normal working pattern

Independently work with patients, family and MDT staff to identify goals as part of the overall care plan

Plan and implement individual treatment and group interventions, in collaboration with the patient, using graded activity to achieve therapeutic goals

Be able to problem solve and judge complex situations that require analysis and interpretation of a range of different options

Provide sensitive and specialised clinical advice to patients and their carers re assessments, treatments and care packages
