Would you like to be part the new Virtual Ward Team?

We are looking for a dynamic, adaptable and forward-thinking individual to the join the Virtual ward team and help develop this exciting new service. Virtual wards allows patients to get the care they need at home safely and conveniently, rather than being in hospital. They enable more people to receive the acute care, remote monitoring and treatment they need in their own home. They can help prevent admissions or allow for an earlier, supported discharge. Virtual wards also provide additional capacity to hospital beds enabling more patients to receive the timely treatment they need.

As part of our Virtual ward team you will support these patients monitoring and treatment that they need in their own home.

You will work alongside and communicate effectively with the experienced Multidisciplinary team in supporting them to deliver excellent patient care across Central Lancashire.

As the role will allow you to work within a hospital and community setting a full UK driving license is required.

This post would be ideal for someone looking to work in a new and innovative service, to support the expansion and development of new pathways in this integrated virtual ward service.

Deliver high quality, compassionate care under the direction of a Registered Nurse (or other registered care professional dependent on setting) with a focus on promoting health and independence

Have proficient attitudes and behaviours compatible with NHS Values

Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual

Carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of a registered nurse or other registered care professionals dependent on setting

Ensure accurate and up-to-date patient details are maintained on patient information systems such as FLEX and Docobo (patient monitoring system)in line with Trust Information Governance policy.

**Duties to include, but not limited to**:
Contribute to the assessment, implementation and evaluation of care to meet patients’ needs in accordance with agreed local and national standards and evidence based guidelines under supervision

Support and provide patient education and health promotion

Support the delivery and co-ordination of care, through appropriate professional practice

Contribute to the management of risk and maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines

Assist in setting, monitoring and maintaining standards of care to improve the patient experience and outcome of care.

Communicate all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care

Maintain accurate and complete records of all care activities, in a timely manner, adhering to local and professional standards for documentation

Act as a role model, presenting a positive image of self, team and the organisation

Promote equality in care and practice by recognising, respecting and meeting the needs and choices of individuals

Ensures environment of care is safe, well equipped and compliant with standards for hygiene and cleanliness.

Maintain and develop own professional competence and skills within scope of role

Actively participates in the appraisal and Personal development planning process as reviewee

Proactively supports Trust wide or local changes and developments in nursing practice e.g. intentional rounding, alcohol screening as directed etc.

Actively try to resolve issues and complaints at local level and then report the outcome to line manager.
