**JOB DESCRIPTION - ADVANCED PRACTITIONER**
**Job Summary**:
To work within own professional boundaries, to provide high quality, safe, cost-effective care for patients. Working as an autonomous nurse, assess patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. Prescribe required treatments and medications in line with practice policies and protocols
The post holder will report to the GP Partners clinically and Practice Manager
**Duties & Responsibilities**:
Following practice policies, protocols and guidelines, duties will include:
Clinical:
- Provide surgery consultations and telephone consultations for patients presenting with minor ailments and chronic problems, to establish a diagnosis and management plan using history taking, physical examination, problem solving and clinical decision making
- Telephone triage patients requesting home visits, using history taking and telephone consultation skills, to establish appropriate management
- Provide home visits to housebound patients to assess and provide appropriate management of acute and chronic conditions
- To prescribe safe, effective and appropriate medication as defined by current guidelines and within own professional competencies
- Provide care and treatment to patients with chronic diseases through pro-active review and management of the condition focusing closely on individual management to improve health care and compliance
- Opportunistic health promotion, particularly in relation to diet, exercise, smoking and alcohol and providing the appropriate preventative health advice and referrals where necessary
- Provide general and specific health screening within agreed protocols with referral to relevant departments where necessary
- Work with patients in order to support compliance with and adherence to treatments providing information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Work closely with the practice team to ensure high QOF / QIF targets are achieved and other local enhanced services provided by the practice
- Assist with vaccination campaigns providing appropriate and timely vaccinations including flu, pneumovax, shingles, and Covid vaccinations
- Administer injections as required to patients, for example B12
- Suture removals and minor wound care
- Act as infection control lead for the practice and ensure appropriate audits, protocols and policies are updated in line with required national standards
Other Responsibilities within the Practice:
- Work as an effective and responsible team member, supporting other members of clinical and non-clinical staff in a flexible and approachable manner
- Awareness of and compliance with all relevant practice policies/guidelines, eg prescribing, confidentiality, data protection, health and safety
- A commitment to learning and audit to ensure evidence-based best practice
- Contributing to evaluation/audit and clinical standard setting within the practice
- Contributing to the development of computer-based patient records
- Attending training, practice meetings and events organised by the practice or other agencies, where appropriate
- Meet all tight timescales/deadlines for audits and written returns to ensure that the practice meets quality standards and receives the designated funding (e.g. annual QOF audit etc)
Teaching & Training:
- Participate in teaching and training
- Participate in the induction of new staff, students and other attached professionals as necessary
- Receive review and appraisal within the practice, and training as necessary to update skills and knowledge
- Participate in the educational and training events of the practice

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff respect their privacy and act appropriately
- In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential
- Information relating to patients, carers, colleagues, other healthcare workers of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal data and sensitive data
