**Job overview**:
Tudor Court is a 24-bed unit in the Community offering rehabilitation / reablement with the aim of a safe and timely discharge home - our aim is to do this within 14 days. Patients are admitted from Hospital or the Community. Tudor Court is a residential unit - there are no Nurses on site - clinical tasks are not performed by staff employed on the unit.

Full and part time hours considered. All posts are rotational and you will be required to work days/nights/weekends and Bank Holidays.

**Main duties of the job**:
To support patients with all aspects of daily living, including washing, dressing and toileting focusing on re-abling patients to return home.

Supporting patients with mobility/therapy practise, including moving and handling and use of equipment. Helping with eating and drinking, social activities, emotional and wellbeing support.

You will work as part of a Multi-Disciplinary Team including Therapists, GPs, Pharmacy and support staff. You will be required to follow care/therapy plans and keep records of evaluations/outcomes throughout your shift.

General Housekeeping duties - keeping the environment safe and free from clutter.
