**Enhanced DBS required for this role.**
**Hybrid - being able to travel to locations in Northamptonshire.**
**This post holder will be working within the Supported Living Review Team that will be working closely with Adult Social Care colleagues to review all Supported Living support packages commissioned by the client.** Key responsibilities:

- Work with social care provider to understand staffing within shared accommodation settings and analyse the number of staff required to meet this individual’s outcomes and to live a safe life.
- Provide a comprehensive monitoring and contract management service that focuses upon the quality of support and care within the organisation commissioned services for younger adults and community prevention.
- Ensure best practice, and the safeguarding of individuals and maintain individual health and wellbeing.
- Fully record all contract activities, agreed action plans, safeguarding concerns and remedial actions.
- Take the lead in developing and disseminating good practice across providers through provider forums and other sources of communication.
- Work with the allocated commissioning manager meeting providers on a one to one and group basis.
- Ensure that all contracted providers provide information as required in performance indicators and to contribute to the development of any new procurement or monitoring process carried out by the Commissioning, Contract, and outcomes Team.
- Ensure that Contracts are evaluated and monitored either virtually or where the need arises in person.

- Experience working in a health and/or social care setting.
- Experience of and ability to work in partnerships to improve services.
- Knowledge and experience of good Adult Care Services and how these are achieved and maintained.
- Understanding of the legislative framework.
- Have excellent written and verbal communication skills.
- Able to work with a range of partner organisations.
- Ability to measure performance and ensure compliance.
