The overall aim of this role is to assist in improving cancer patient experience. This will be achieved through supporting personalised care and ensuring patient needs can be met, from diagnosis, throughout the cancer pathway, allowing them to live life as fully as possible. They support our Clinical Nurses Specialists (CNS)'s with less complex calls and needs.

A key aspect of the role involves daily and direct communication with patients, relatives, carers, and other health and social care professionals. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity.

You will coordinate access to the right information and education resources to support people in making decisions about aspects of their own care, enabling independence, and develop a partnership approach to working in order to empower the patient and carers.

The role also requires an element of administrative and co-ordination skills as part of cross covering the Pathway Navigator Role.

The role would also be suitable for those looking for a secondment opportunity. Interest from Assistant Psychologists, Junior Social Workers, HCA's etc. will be welcomed. Clinical supervision will be supplied.

The scope of the role will vary, though the main objectives are:

- Provide cover to the paediatric oncology call service and respond to patient queries in a professional and efficient manner. Follow the Standard Operating Policy for clinical issue escalations
- Book and outcome cancer CNS appointments on PIMS in accordance with the agreed process
- Set up Electronic Health Needs Assessments (eHNAs) and contact checklist with patients, book and outcome appointments for completion of care plan with cancer CNS. This may involve telephone or face to face consultations with our patients depending on the setting
- Support patient events and real time survey capture

The Band 4 Paediatric Oncology Support Worker will demonstrate a level of skill and understanding which enables them to undertake specific tasks delegated by the team lead and in accordance with relevant procedures, guidelines and policies. They will undertake both routine and specially identified tasks for which they have been trained and assessed as competent.

Key

**Responsibilities**:

- Contribute to the eHNA and the development of an individual care plan under the direction of the CNS
- Provide general telephone advice, signposting patients/carers to other sources of support as appropriate
- Coordinate appointments or investigations to fast track patients through the PIMS system
- Liaise with the MDT coordinators to ensure a seamless service for patients and smooth transition through the patient pathway
- Identify patient/carer general information needs and signpost to the Macmillan Information and Support centre as appropriate
- Support the delivery of patient and carer training and education, including the use of equipment
- Advise patients on individual self-care management support and education as necessary to non-complex patients, including how to self-assess
- Encourage and support healthy life style choices
- Support the development of eHNAs and care plans at the end of treatment and the initiation of end of treatment summaries
- Ensure flexibility as a member of the team and respond to the changing needs of patients and the service
- Provide a vital link between patients and the CNS, highlighting patients that require specialist assessment.
