- Rota worked out fairly so you dont work every weekend.
**Salary -** £10,374 plus 3% employer’s contribution to group personal (stakeholder) pension scheme after 3 months employment

Our Organisation
Age UK Gateshead is a charitable organisation run by people for people. At Age UK Gateshead we deliver services for older people within the different communities across Gateshead.
This role
The Main purpose of this role is to support an individual after discharge from hospital to avoid readmission. This will be done by aiding with basic house hold tasks, cleaning, shopping etc then ensuring longer term support is in place. The service will also provide and direct to services covering support in areas such as, utilities, food security, hydration and nutritional information, falls prevention, social interaction and much more.
The principle of this bespoke hospital to home service based upon the Esther project (the person at the centre of what we do) all participants will be allocated individuals who will meet them personally at home for an initial assessment, these principles will assure the individual will receive a service that will aid them and their specific needs.
The role will provide initial support to improve or maintain quality of life post discharge and after major illness and remain in place until you have sourced longer term support.
**This service is a sixn days week service.**
**Responsibilities**:
- Act on referrals received in accordance with timescales, Age UK Gateshead policies and procedures and quality standards.
- Proactively work with an in a local health care setting.
- Maintain accurate records.
- Update client records
- Identify further support, refer to other agencies where appropriate (including AGE UK Gateshead services)
- Obtain essential shopping items initially and encourage support back into usual routine or explore alternatives
- Offer support to facilitate safe and timely discharge home
- Offer support to speed up the discharge process
- Offer support to prevent an unnecessary admission into hospital
- Complete client case reviews ensuring they are completed at agreed times, check support and interventions have been provided and action follow-on referrals as a result of the review
- Ensure a person has all provisions they require at home upon discharge (heating, lights, snack, food provision etc.)
- Support clients to remain as independent as they are able to be
- Learn and deliver a service in line with the Esther project principles putting in longer term soloutions.
- Promote and raise awareness of Age UK Gateshead’s Hospital to home service and other relevant Services.
- Work with the wider team to assure quality of service an optimal care.
- Travel from home to home to deliver the service
- Respond to relevant queries from referrers, clients and members of the public.
- Develop and maintain contacts with relevant professionals
- Keep abreast of developments in relevant legislation and good practice guidance.
- Work in accordance with Age UK Gateshead guidelines and with commitment to the ethos and principles of independent advocacy.
- Be an active participant in the wider Age UK Gateshead team and seek to work with all Age UK Gateshead colleagues to deliver our strategy, maintain quality and embed the values of the organisation.
- To Support other Support worker Roles when necessary.

Person specification and Qualifications
Experience and qualifications;
- Experience caring for vulnerable people.
- Experience in a home care environment
- Experience & qualifications in Health and social care

- Have experience of working with people with multiple health challenges
- Be passionate about care and making a difference.
- Be well-organised with an ability to handle a busy workload and prioritise effectively;
- Understand the needs of older people and address their needs on an individual basis

- Be able to work as part of a team an independently
- Be able maintain good time keeping and stick to a schedule.
- Be able to thrive under pressure and bring people together.
- Understand the importance of confidentiality and maintain appropriate confidentiality at all times
Use of your own car will greatly assist you in this role
Other Information
- 25 days holiday p.a. pro rata (plus public holidays)
- 3% contributory pension scheme after 3 months
- Salaries are paid direct into your bank account on 21st of each month
- This post will require an enhanced DBS disclosure
