This post is a Band 3 Rehabilitation Assistant, working within the Specialist Palliative Care team and Hospice at Home service within Gateshead.

The post holder will work effectively within a multi-disciplinary/agency team visiting patients who have been referred to the Specialist Palliative Care Team and Hospice at Home service. Patients and families / carers will have potentially complex needs and require a flexible, compassionate and caring approach to interventions appropriate to their diagnosis and prognosis. Specific focus on relaxation, breathing techniques, quality of life enhancing approaches, legacy / memory work for patients and families.

There will be planned activity to provide hands on care and holistic assessment to promote independence and offer improved quality of life in the patients preferred place of care.

This new post has been created to support the Specialist Palliative Care and Hospice at Home teams fulfil their objectives, ensuring the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients’ health and wellbeing, within the community setting.

Secondment opportunity will be considered.
- To work closely with the nursing, physiotherapist and occupational therapists to support programmes of rehabilitation, reablement, recovery and health promotion thereby reducing the need for unnecessary hospital admission/ attendance and long-term care.
- To ensure that the highest level of patient care is achieved at all times in the most effective manner
- The duties will be carried out in the community setting under the supervision and guidance of a Therapist/Nurse however the post holder will be expected to work alone with access to professional advice and guidance when required.
- Support the wider therapy team to follow up a range of therapy interventions to patients within the community setting.
- To support the point of contact ensuring patients receive care at the right time, right place, by staff with the right skills.
- Support the Specialist palliative care team and Hospice at Home teams lead in the development of any new ways if working that will ultimately improve patient care as and when required.

- GCSE’s at grade C/4 or above in Math’s and English Language
- NVQ level 3 in Health or Social care specialty or equivalent knowledge/experience (or working towards)
- Full Driving Licence
- Experience of working in a Healthcare/Social Care Environment.
- Experience with working with older people to support rehabilitation needs
- Experience with Adults with long term conditions or end of life care needs.
- Experience of working in Hospice setting
- Experience of equipment assessment of ordering via Gateshead GES systems
- Experience of working collaboratively with a wide range of staff
- Experience of working independently.
