18 Week Patient Pathway Administrator

Patient Access Services is a busy and diverse area that covers a range of different areas across the Trust including Outpatient and Inpatient appointments/operations booking and managing the Trusts Referral to Treatment (RTT) pathways. We are looking for two people who are interested in RTT validation full time.
The post holders’ main focus will be to track patients across their 18 Week pathway, ensuring the service provides a patient centred approach at every stage of the patient journey, and ensuring potential breaches of RTT (referral to treatment) waiting time targets are avoided or escalated to the relevant manager.
Applicants will need to be a good team worker as well as able to work independently. They will need to be flexible, and able to use their own initiative working proactively with colleagues to ensure waiting time targets are achieved.

The post holder will ensure that patients waiting for treatment on an 18 week RTT pathway are validated, and their treatment pathways are expedited in line with service and patient expectations. They will ensure that all potential breaches of the target are escalated and managed appropriately by working closely with the administrative teams. They will work with service managers and clinicians to ensure patients are treated in a timely manner.

Interrogate hospital systems including Lorenzo, Evolve, Soliton, Ice, Medical Records and BI to monitor and record every event in the Patient Pathway.
To ensure that mandatory data items are accurately obtained, checked and recorded ensuring compliance with information and data quality standards in order to support divisional operational teams to achieve a high standard of data quality and completeness.
To provide training and support to administration, clinical and ward teams with regards to referral to treatment pathways and data quality.
Work flexibly and provide cross cover for colleagues in the Team.
Working in line with the Trust’s Local Access Policy and national guidelines.
Assist in completing the validation of monthly and weekly submission data.
**Referral to Treatment Validation**:
Support the investigation of long wait breaches, e.g. 30 week and above.
Collect data retrospectively, as and when required to help inform RTT related service reviews or breach analysis.
Communicate with, and train, clinical and administrative teams on RTT rules and guidance, including clock start and stop rules.
Co-ordinate and carry out manual validation of data prior to submission of weekly and monthly RTT reports - by checking details in EPR, referral/clinic letters or other Trust systems.
Provide guidance and support to clinical and administration teams on recording of clock starts and stops.
Work closely with the RTT team leader, RTT Manager for Patient Access and Information team colleagues to ensure that RTT reporting is robust and accurate.
Assist in regular audits to measure the quality of RTT clinic outcomes
Identify long waits or inefficiencies on the RTT pathway and flag to service managers to avoid breaches of the target where possible.
Work with General Managers to help ensure patient pathways are expedited in line with RTT rules, internal operational policies and to ensure that patients are treated equitably and on time.
Weekly updates to the RTT/Outpatient Manager regarding the progress of patient’s pathways and escalating concerns/issues in delays in the patient’s pathway as identified by the diagnostic pathway report.
Gain an understanding of the medical terminology of diagnosis and treatments in order to fully understand the patient’s pathway.
Undertake such other tasks as required by the RTT manager in accordance with the grade and nature of the post.
Work with multi-disciplinary teams to help improve data accuracy recorded within EPR.
Work with bookings teams to help support, monitor and inform booking efficiency in outpatient, diagnostic and inpatient/day case bookings
