**Key responsibilities include**:

- Supporting the delivery of a varied programme of activities
- Monitoring day to day support of the care team.
- Assessing and monitoring the needs of clients to ensure a person-centred approach.
- Developing care plans to meet individual needs.
- Liaising with key partners
- Hold client reviews and support clients to meet their personal goals within their chosen activities/interests.
- Supporting projects and engaging with groups in the community
- Developing individual and group programmes to meet specific objectives
- Maintaining accurate records
- Risk assessments

- Good general standard level of education.
- English GCSE at grade C or above, or equivalent
- Basic knowledge of Health and Safety

**Desirable**:

- Experience of supervising staff
- NVQ3 Health and Social Care
- Knowledge of Direct Payments, In Control, Personalised Budgets
- Experience of using Person Centred Thinking Tools
- Facilitating and motivating individuals and groups within Centres and Community
- Some specialist skills i.e. independent living skills
