Motor Neuro Disease Care Co-ordinator

The MND Peninsula Care and Research Network is looking to appoint a new keen and dynamic band 6 to their service.

The post is a Band 6 job, four days a week, but we are keen to be flexible how those hours are worked. The post is a mix of working from home and in the MND office at Derriford Hospital, but also with a lot of your time spent out on the road, visiting patients, attending clinics etc.

The service covers the whole of Devon and Cornwall but ideally this person will mainly be responsible for covering the Plymouth/South Hams and Torbay part of the patch. However, during leave/sickness you may be asked to go to other parts of the patch too.

The job is initially available until March 2024, but we believe ongoing funding will be made available after this. We are also able to explore options for secondment, but this depend on your current manager being agreeable to this.

You will be working alongside 4 other team members with the aim of providing high quality care to people with MND and their family/carers as well as local professionals supporting these patients.

After induction, you will be organising/being part of MND MDT clinics in Torbay and Plymouth. You will be facilitating MND Multi-Disciplinary Team meetings in these area, as well as Tavistock and South Hams areas.
- To achieve a highly specialist knowledge of this condition, underpinned by theory that will be used to care for this vulnerable patient group from diagnosis to end of life

- Ensure they attend Child Protection training at the appropriate level within the specified time frame.

- Maintain the prevention and control of infection and fully comply with all current Trust Infection Control policies and procedures.
- Take responsibility for any records that they create or use in the course of their duties, in line with the Public Records Act

**Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.**

Clinical;
1. Co-ordinate the specialist advice/care following the diagnosis of Motor Neurone Disease. The care co-ordinator will work within the geographical area covering the Plymouth/Torbay/South Hams areas within the Network. He/she will also provide an advisory service across the whole Network area as and when required, acting as a point of reference in their chosen speciality and specialist advice for patients, families and health and social care professionals. They will be able to provide support in other areas if other team members are not available, in a temporary fashion.
2. Be able to offer specialised advice for patients and their families in their own homes and other establishments that provide care, after necessary training/development. To provide timely support, advice, information, and knowledge about MND to patients and families who can be signposted to various services.
3. To support and maintain professional relationships with the patients and their families, acting as guides, advocates, or critical friends.
4. Use holistic and evidence-based patient centred principles to assess, plan, implement and evaluate highly specialised care programmes designed to maximise the individual’s potential to control and adjust to their condition.
5. The post holder is expected to build and maintain links with local professionals and voluntary services and develop expert knowledge of other local resources that can provide help and support such as benefits and bereavement services. To be aware of referral processes to different teams within their area. Be responsible for all written communication at time of each referral.
6. The post holder will be part of MND multidisciplinary clinics in Derriford and Torbay areas, with the lead clinicians to provide expert advice and support to those living with MND and their families and the professionals involved in their care. They will also aim to support patients admitted to these hospitals
