To be responsible for managing a caseload of clients identified by a registered Occupational Therapist, Physiotherapist and/or Dietician using technical/creative skills to enable clients to maintain independence and well-being. Compile reports on clients occupational functioning and formulate recommendations for specialist equipment and adaptations Under the supervision of a registered Occupational Therapist, undertake activities which enable individual clients to achieve their Occupational Therapy goals, using ongoing continuous assessment and adjusting own clinical interventions to meet the fluctuating needs of the clients. Under the supervision of a registered Physiotherapist, undertake rehabilitation programmes with clients to enable individuals to achieve Physiotherapy goals. Under the supervision of a registered Dietician, undertake rehabilitation programmes with clients to address nutritional goals.

Be responsible for the assessing, issuing and fitting of specialist equipment and the provision of adaptations, aiming to maintain the independence of clients living within their home environments. To undertake follow-up visits to review and monitor the continued suitability of equipment and adaptations. Assist in the implementation, evaluation and modification of Therapy and team interventions. Work effectively as a member of the Intermediate Care team to provide a community based service by undertaking delegated tasks which contribute to the safe and smooth running of the service.

Undertake the adjustment and alteration of specific items of equipment in line with appropriate health and safety guidelines. To be responsible for planning own day to day allocated workload including reacting to referrals within agreed time scales to prevent clients being admitted to hospital or other care settings. To work independently in a variety of community settings, where the environment is previously unknown and may pose a risk to personal safety including exposure to varying degrees of cleanliness, space and temperature To be exposed to body fluids i.e. sputum, faeces, vomit.

To be aware of and act appropriately with regards to own and others personal safety when working in the community setting. To carry other members of staff as passengers and transport vital Occupational and Physiotherapy equipment in order prevent unnecessary hospital admission and to facilitate a timely hospital admission. To be a car driver and drive as required throughout the day in all weather condition. To be flexible to work in a variety of clinical settings in Intermediate Care, and have the necessary competencies and skill required to undertake work as designated by an Occupational Therapist, Physiotherapist or Dietician.

To assist the qualified Occupational Therapist, Physiotherapist and/or Dietician to carry out rapid, specialist assessments in the community in order to prevent unnecessary hospital admissions. To carry out up to 6 weeks rehabilitation, as directed by the Therapist with clients in their own homes or in a relevant Intermediate Care setting, in order to maximise the clients independence.
