Job Summary:
We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for assisting in the coordination and management of care services for our clients. Your role will involve working closely with healthcare professionals, clients, and their families to ensure the delivery of high-quality care.

**Responsibilities**:

- Collaborate with healthcare professionals to develop and implement care plans for clients
- Coordinate and schedule appointments, treatments, and follow-up visits
- Communicate with clients and their families to provide support and answer questions
- Monitor client progress and update care plans as needed
- Ensure all necessary documentation is completed accurately and in a timely manner
- Maintain confidentiality of client information
- Assist in the coordination of community resources and support services
- Provide emotional support to clients and their families

Qualifications:

- High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred
- Proven experience in an office or administrative role
- Excellent organizational skills with the ability to multitask and prioritize workload
- Strong communication skills, both written and verbal
- Proficient in Microsoft Office Suite (Word, Excel, Outlook)
- Ability to work well independently as well as part of a team
- Compassionate and empathetic nature
