Advanced Practitioner Team Lead

The post holder is the practitioner expert and is required to have a highRespiratory specialist knowledge ensuring the provision of effective and efficientcare. At this level the post holder will be expected to work as a highly skilledsenior practitioner and be the expert.Primary focus of the role is to work as an advanced, autonomous practitionerproviding patient-centred care, encompassing the skills of assessment,examination, diagnosis and treatment of patients.The post holder will; Provide timely responsive interventions which at times will be at shortnotice and provide improved outcomes for patients by improving timelinessand consistency of care throughout Powys. Exercise a high degree of personal professional autonomy and make criticaljudgments to satisfy the expectations and demands of the role, within adefined framework. Expected results are defined but the post holder willdecide how they are best achieved and is guided by principles and broadoccupational policies.
Expected to work at an advanced clinical level withintheir level of competence and to work flexibly as part of the team. Act as a practice leader, managing their own workload and work acrossprofessional, organisational, agency and system boundaries to improveservices and develop practice. They will participate in networks, locally,regionally and nationally. Assess and manage risk and proactivelychallenge others about risk.
Work in primary and intermediate care settings to train staff to help peoplewith Respiratory long-term conditions to develop management skills toimprove their health and wellbeing. This will involve working with PrimaryCare and the extended practice and Community Resource Teams; workingwith clinical specialists to develop and run condition specific modules andprogrammes and providing support to staff in Day Hospitals and Health andWellbeing Centres throughout Powys. Plan and manage a caseload of people who have long term healthconditions and are at significant risk of further deterioration in their healthand help them to implement self-management strategies to improve theirhealth and wellbeing and signpost as appropriate. Support and train other colleagues working with people with long termhealth conditions.
