**About the role**:
Continuing Care and Complex Care Practitioner role across both children’s and adult services

This is a fairly new team where you can assist in the team’s continued growth and development.

You will work closely with colleagues across both social care and health in ensuring that Continuing Health Care and Continuing Care for children and young people Legislation and Frameworks and adhered to.

You will need to have an interest in Continuing Health and Continuing Care, want to develop your knowledge, participate in delivering joint training with our health partners, and support your colleagues.
