We are looking for dedicated, enthusiastic and motivated team members to join our developing service focussing on preventing, reducing and delaying the need for care and support and promoting wellbeing and independence. Social Care Practitioners are required to work collaboratively with colleagues from different teams, disciplines and partner organisations in order to provide a seamless service to individuals/families/carers. They will help to protect vulnerable adults from harm or abuse and support people to optimise their choices in order to live as independently as possible. They will work with communities, individuals and families to promote their personal strengths.

The Community Facing and Hospital Facing teams work under the leadership and management of the Independence and Wellbeing Service (IWS) Team Managers. The teams provide the first response for Adult Social Care providing information and advice, connecting to other agencies, considering assessments of need and working with the public, other professionals and external agencies - Including responding to Safeguarding Concerns. You will deliver positive outcomes for residents.

We have the chance to develop within a multi-disciplinary context, share information and together deliver better outcomes. Opportunities exist around the new pathways supporting people to be safely discharged from hospital and also avoid admission to hospital. Working with and across our primary care networks, we are finding better options to provide the care and support that helps people to stay safe and supported at home.

The office base for these roles are in Truro and St Austell and the community within which we work is Mid Cornwall - We are evolving ways of working meaning that the role requires agility and the ability to work remotely (e.g. from home/other locations).

We are recruiting Social Care Practitioners to join a forward thinking service which is made up of a combination of different professionals with a variety of knowledge, skills and experience.

We have 1 full time position in the Community Facing Team currently available.

The work environment is positive, busy and one where staff are provided with regular support, supervision and training, as well as, career progression and the opportunity to be involved in the development of the team and wider service.
