We are recruiting a Senior Practitioner to join the Community Facing Team within a forward thinking service which is made up of a combination of different professionals with a variety of knowledge, skills and experience.

The Social Workers within the Community Facing and Hospital Facing teams work under the leadership and management of the Independence and Wellbeing Service (IWS) Team Managers. The teams provide the first response for Adult Social Care providing information and advice, connecting to other agencies, considering assessments of need and working with the public, other professionals and external agencies - Including responding to Safeguarding Concerns. You will be expected to use evidence-based practice to deliver positive outcomes for residents.

The work environment is positive, busy and one where staff are provided with regular support, supervision and training, as well as, career progression and the opportunity to be involved in the development of the team and wider service.

Within IWS, the Social Work focus is initial contact and assessment work to gather information and consider the most appropriate next steps - This would include providing information and advice, signposting, connecting to other agencies, working closely with the voluntary sector, progressing to reablement or undertaking an assessment with a focus on maximising independence and wellbeing through utilising social care and occupational therapy pathways before going to care and support planning.

Social Workers within the team need to promote self-learning and keep knowledge and skills up to date, working to Adult Social Care practice standards in line with the national Professional Capability Framework. Sharing information and expertise, you can help others in your team grow and achieve their goals.

We have the chance to develop within a multi-disciplinary context, share information and together deliver better outcomes. Working with and across our primary care networks, we are finding better options to provide the care and support that helps people to stay safe and supported at home.

The office base for these roles are in Truro and St Austell and the community within which we work is Mid Cornwall - We are evolving ways of working meaning that the role requires agility and the ability to work remotely (e.g. from home/other locations).

Cornwall has a Net Zero carbon strategy to reflect our priories and responses to the climate emergency.

This is a public/customer-facing role, where the statutory English language requirement for public sector workers applies.

Whilst the location/work-base for this role may have been specified our evolving ways of working means that the role may require you to also work remotely (e.g., from home/other locations). We will discuss this with you during the recruitment process.

**About You**

We are looking for someone dedicated and motivated to join our developing service focussing on preventing, reducing and delaying the need for care and support and promoting wellbeing and independence. You will need to be self-motivated with the ability to prioritise, work under pressure and stay focused in this high paced and evolving environment.
