An opportunity has arisen for a registered nurse with experience of leadership in end-of-life to join the BNSSG - Funded Care team.

This is an exciting opportunity to lead the Fast Track service and deliver clear objectives with a dedicated team of staff supporting referrals from across the BNSSG demographic.

This role involves leading the team to coordinate referrals from acute trusts and the community, liaising with health professionals to establish clients’ needs and supporting brokerage to identify packages of care or placement to meet those needs.

The ongoing support offered to the individual and their family may highlight welfare concerns, quality assurance issues &/or the need for rapid intervention as situations change. Therefore, you will need to be aware of patient risk, the services available across BNSSG and be a confident decision maker in order to balance the need of the patient with resource management to ensure the provision of care is both person centred and sustainable.

The role will give scope for the individual to identify and lead on service improvements and engage with senior management in system working. Therefore, an attention to detail and the ability to work collaboratively with other agencies is essential and experience of leadership in such a manner is desirable.

Our team is based in the heart of Bristol covering the BNSSG demographic so this role will involve travelling across the area and the post holder will be expected to have their own transport.
