Ncgpa Lead Social Prescribing Link Worker

As a Lead Social Prescriber, you will play a crucial role in enhancing the impact of social prescribing interventions within our organisation. You will be responsible for supporting and guiding other link workers to improve self-care, the quality of relationships, and service development while also assisting individuals in accessing the support they need to improve their overall wellbeing. Additionally, you will be instrumental in identifying funding opportunities and preparing successful bids to secure resources for our social prescribing initiatives.
**Responsibilities**:
**1. Leadership and Guidance**: Provide leadership and mentorship, supporting effective social prescribing services to clients. Offer guidance on best practices, case management, and complex client situations.
**2. Service Development**: Collaborate with stakeholders to develop, refine, and expand social prescribing services. Identify gaps in service provision and propose innovative solutions to address community needs.
**3. Self-Care and Wellbeing Support**: Work closely with link workers to empower individuals to take charge of their own wellbeing. Provide training and resources to enhance self-care strategies and positive lifestyle choices.
**4. Quality Assurance**: Implement and monitor quality assurance processes to ensure consistent and high-quality service delivery across the social prescribing team. Conduct regular audits and performance evaluations to identify areas for improvement.
**5. Relationships and Partnerships**: Establish and maintain strong relationships with partner organisations, healthcare providers, community groups, and local authorities. Collaborate with these stakeholders to ensure a coordinated approach to social prescribing and maximise available resources.
**6. Funding and Bidding**: Identify funding opportunities from various sources, including grants, foundations, and public sector funding. Lead the development and submission of compelling funding proposals to secure resources for social prescribing initiatives.
