**An exciting opportunity has arisen for a permanent Adult Social Care Worker to join a well-established and supportive Adult Social Care Team based in Castlewood, Clevedon with hybrid working available. The team covers the more rural north patch of North Somerset including the towns of Clevedon, Portishead and Nailsea as well as all the surrounding villages.**

**We welcome applicants who may have a background in Health and Social Care but of equal importance is experience of transferable skills related to the job description.**

**About the role**

The Adult Social Care Workers are valued for their skills, knowledge and experience they bring to the services. They are key to delivering an outstanding service for the most vulnerable residents in North Somerset.

Working within key legislative frameworks including the Care Act 2014 and the Mental Capacity Act 2005. As an Adult Social Care Worker you will be undertaking Care Act Assessments, co-producing care plans with individuals, identifying their strengths and person-centred outcomes. You will undertake mental capacity assessments and be involved in best interest decision making. You will be holding a manageable caseload supported by Social Workers and Senior Social Workers.

We will utilise your transferable skills and provide training to enable you to promote the rights and choices of people with care and support needs, you will support people to identify and build upon their existing assets as well as access new resources to meet their identified outcomes. This can include supporting positive risk to promote independence, safety, and wellbeing of people with care and support needs. You will need to be confident in Information technology skills including documentation in databases.

Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.

**This position is currently based at our Castlewood offices in Clevedon. Please be advised that this office will be closed by 2025. Consultation is currently underway to determine the new location for colleagues based in Castlewood. This will be at Weston-super-Mare or within North Somerset.**
