An exciting opportunity has arisen for a permanent full-time 37 hours Adult Social Care Worker to work in the Mental Health and Wellbeing Integrated care Team (MINT).
The MINTs were set up in recognition of the ‘gap’ in mental health services which exists between primary care (GPs) and secondary care (services currently provided by the Avon and Wiltshire NHS Mental Health Partnership trust); with the aim of providing a range of proactive and preventative interventions to prevent the escalation of mental health needs.
You will be joining a very forward-thinking supportive locality team and be working day to day as part of the MINT. The MINT will be person centred and provide and coordinate a high quality, holistic response to an individual’s care needs, for people with a wide range of mental health conditions. You will work alongside people to provide with information and advice to enable them to use their own skills and assets to keep them well or to support their recovery. You will be working alongside other professionals (including nurses, psychologists and the third sector) in the MINT to engage with people to reduce support and maximise independence related to their mental health.
As an Adult Social Care Worker in this team, you will bring your skills and experience in strength-based person-centred practice and social care to support as part of the broader Multi-Disciplinary Team. The work will involve completing Care Act 2014 assessments on occasions when these may be necessary. Drawing up care plans with individuals, identifying their strengths and person-centred outcomes. You will complete Mental Capacity Assessments under the Mental Capacity Act 2005 and be involved in Best Interest Decision making where appropriate.
The MINT continues to develop, and this role will be an opportunity to gain skills and knowledge in mental health, working alongside clinical staff – you will continue to have the support of a social care locality team to keep up to date with developments and share learning from your experiences.

This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.
Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.
