Job Summary:
Tripod Partners is seeking a skilled and compassionate Social Worker to join our Adults Community South Team. As a Social Worker, you will work closely with adults in the community who may have physical disabilities, learning difficulties, or mental health needs, ensuring they receive the necessary support to live independently and with dignity.
Key Responsibilities:
- Conduct needs assessments and reviews under the Care Act 2014, ensuring that individuals are supported in a person-centred, strengths-based way.
- Develop and implement care and support plans that promote independence and well-being.
- Collaborate with a range of professionals, including healthcare providers, housing officers, and voluntary organisations, to coordinate services and interventions.
- Provide safeguarding support and manage risk to ensure vulnerable adults are protected from harm and abuse.
- Work in partnership with individuals, their families, and carers, empowering them to make informed decisions about their care.
- Promote a culture of equality, diversity, and inclusion in all aspects of your work.
Requirements:
- Degree in Social Work (or equivalent qualification).
- Registered with Social Work England.
- Experience working with adults in a community-based setting, including managing complex cases.
- Strong knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults procedures.
- Full UK driving license and access to a vehicle for community visits.
- Experience in a local authority adult social care team.
- Knowledge of Bristol's local services and community resources.
