Do you have lived experience or work experience with people with a learning disability, autistic people or both? Are you a good communicator and able to work closely with a range of multi-agency partners?

We are expanding our team and this is a fantastic opportunity to join theHumber and North Yorkshire Keyworker Service and work with people with a learning disability, autistic people or both up to the age of 25 who are at the highest risk of being admitted to a mental health hospital, and their families/carers. There are two posts available.

**You should**:

- Have experience of people with a learning disability, autism or both.
- Have empathy, compassion, respect, commitment to ensuring the rights of people with a learning disability, autism or both are protected, be positive and solution focused and be committed to joined-up collaborative working.

**This post holder will**:

- Manage a caseload of people who have been identified as requiring a Keyworker.
- Facilitate the provision of emotional and behavioural support and informed advice to people and their families/carers based on knowledge and experience of working with people, families and carers with complex needs.
- Provide information and support to ensure that personalisation approaches are embedded in their plans and that creative solutions are considered to meet the individual’s needs.
- Create and encourage a culture which prioritises keeping people within the home and their local community by communicating with a range of teams, services and staff from a range of statutory and non-statutory.
- Build and maintain positive, trusting and appropriate relationships with people with a learning disability, autism or both and their families/carers.

Training will be provided to Keyworkers to ensure that they have detailed knowledge and understanding of processes relevant to the role.
