Job Summary

We are seeking a highly skilled and compassionate Support Worker to join our team at Victoria House, a Good-rated assessment centre and rehabilitation unit. As a Support Worker, you will play a vital role in providing high-quality care and support to our service users, enabling them to regain their skills and live fulfilling lives.

Main Responsibilities

  • Provide individualised support to service users, promoting their independence and autonomy.
  • Contribute to creating a homely and pleasant living environment, fostering positive relationships with service users and colleagues.
  • Facilitate daily routines, including personal care and promoting high standards of personal hygiene.
  • Participate in the development and implementation of service user support plans, risk assessments, and programmes.
  • Implement individual support plans, highlighting any issues or concerns to the Team Leader to ensure quality services are delivered.
  • Enable service users to access and utilise resources and activities within their local and wider communities.
  • Act as a Keyworker for designated service users, liaising closely with relevant staff to ensure quality services are delivered.
  • Report any enquiries, issues, or concerns to the line manager, providing information to enable them to take appropriate action.
  • Ensure all relevant documentation is completed accurately and effectively in a timely manner.
  • Attend and contribute to annual reviews, multi-disciplinary meetings, and Person Centred Planning meetings.
  • Contribute to daily handovers between shifts, ensuring effective communication between staff.
  • Support and administer medication following relevant training, in line with the support plan and individual guidelines.
  • Support and facilitate financial transactions for service users and the Trust, in line with the individuals support plan and company policy and procedures.
  • Support service users' health and wellbeing, including attending appointments and promoting healthy lifestyles.
  • Work in accordance with the rota system, including working unsociable hours at weekends and some bank holidays.
  • Undertake any reasonable travelling requirements to fulfil the tasks and responsibilities of the role.

Person Specification

We are looking for a highly motivated and compassionate individual with excellent communication and interpersonal skills. The ideal candidate will have experience of supporting a service through a phase of growth and improvement, and be skilled at supporting staff through tools such as supervisions and observations to give excellent quality care.
