Job description

**Summary of Role**

Working as part of a team, you will be working with people between the ages of 18 and 64 year of age who have Mental Health issues, substance misuse, self-harm, personality disorder or behaviours which may be perceived as challenging not excluding those with a history of or identified risk of offending.

Your role will be to offer these individuals support in day to day activities to achieve their maximum potential and to live as independently as possible. To provide the highest quality of care and support to all service users ensuring that they are supported in achieving their aims, goals and ambitions.

**Main Duties and Responsibilities**

**Leadership**

Provide leadership to the team acting as a positive role model at all times.

Lead and delegate tasks and responsibilities within the shift, ensuring that all staff members provide support in line with individuals support plans and identified needs.

Support, motivate and mentor the staff team as required and ensure good team working.

Observe staff to ensure that their performance is of a consistent standard.

Ensure that the full range of duties for each shift worked are appropriately and fairly allocated and completed within a reasonable time frame, (e.g. cleaning, audits, medication etc.) and that all necessary paperwork is completed.

Ensure that the handover sheet is completed and kept up to date and that a comprehensive handover is carried out at the changeover of each shift.

In the absence of the manager or the assistant manager deal with calls requiring information and ensure that messages are left for the manager / assistant manager as required.

**Support**

Work with the management team, service users, families and other professionals, to prepare and implement individual personal support plans and relevant group training programmes to ensure that service users are supported in developing their own skills to their full potential.

Be responsible for ensuring that the care and support services are provided to the highest standards in line with contracts, person centred support plans, best practice and having regard to legislation and regulation.

Provide continuity and constant support including practical, active, and emotional support to the service users which reflect individual changing needs.

Recognise and deal with adult protection issues and the duty of care, and to comply with policies and procedures for the Safeguarding of Vulnerable Adults.

Effectively communicate with the service users in a means by which the individual can understand the information being conveyed and maintain actual and factual records at all times.

Read, understand and work in line with support plans.

Report any areas of risk to the line manager / Manager.

Observe and report on psychological, emotional and physical well-being at all times.

Assist, support and encourage service users to become integrated members of their local and extended communities, promoting and supporting social and physical presence in community based facilities e.g. educational, occupational, religious, cultural, and social and leisure.

General housekeeping e.g. cooking, shopping, domestic duties and helping service users to maintain all aspects of their domestic lives.

Work alongside service users to promote a healthy diet.

Supervising and monitoring service users in self-administration of medication as prescribed.

Administering medication from a monitored dosage system if a service user is assessed as not able to manage safe self-administration.

Follow company policy and procedure in the safe storage, recording and disposal of medication.
