- The role holder will work as part of the team delivering the Early Years Delivery Model. They will hold responsibility for working as part of a multi-agency team and as part of The Team Around the Child. The role holder will work at front line level, at a high quality and when necessary assertively, and use a whole family approach. They will be responsible for delivering identified interventions with families where the child/ren are under the age of 5 years and have been identified as requiring a package of support to improve outcomes and achieve their milestones in development - either at a universal level or at level 2 on the Multi Agency Threshold document, these include: health, social, education and environmental.

The role holder will be responsible for delivering identified interventions, supporting families alongside other services and professionals to ensure child/ren and parents which are identified as a priority to access the full range of services via the Sure Start Children’s Centres and have a role in coordinating provisions and interventions.- Work and support colleagues and key partners to effectively help deliver a timely and professional service. Participate positively in team meetings, team and service development.. Ensuring collaboration and good communication and information sharing both verbally and written with all parties.
