Overview: We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in assisting clients and their families in navigating the healthcare system and accessing the necessary resources and support. This is a rewarding opportunity to make a positive impact on the lives of others. Duties: - Coordinate and manage care plans for clients, ensuring that all necessary services are provided - Conduct assessments to determine clients' needs and develop individualized care plans - Collaborate with healthcare professionals, including doctors, nurses, and social workers, to ensure coordinated care - Assist clients in scheduling appointments, arranging transportation, and accessing community resources - Provide education and support to clients and their families regarding healthcare options and available services - Maintain accurate and up-to-date documentation of client interactions and care plans - Monitor client progress and make adjustments to care plans as needed - Advocate for clients' rights and ensure their needs are met Skills: - Excellent communication skills, both verbal and written - Strong organizational skills with the ability to prioritize tasks effectively - Compassionate and empathetic approach towards clients - Ability to work collaboratively with a multidisciplinary team - Proficient in using office software (e.g., Microsoft Office Suite) - Detail-oriented with strong problem-solving abilities - Knowledge of healthcare systems and resources We offer competitive compensation packages, including benefits such as health insurance, paid time off, and professional development opportunities. **Salary**: £24,000.00-£27,000.00 per year
