About Our Job Opportunity

Unity Healthcare Recruitment Ltd is seeking a skilled Nurse Care Coordinator to join our team in Shropshire. As a Nurse Care Coordinator, you will be responsible for coordinating care delivery and supporting patients to maintain independence and achieve their healthcare goals.

The ideal candidate will have experience in community nursing and excellent communication skills. You will work collaboratively with other healthcare professionals to ensure coordinated care and support patients to achieve the best outcomes.

Key responsibilities include:

  • Clinical Care: Providing health education and advice to patients and carers to support self-management and promote independence.
  • Community-Based Care: Using digital tools and systems to plan, record, and report on care delivery in line with organisational policies.
  • Team Collaboration: Acting as a patient advocate, ensuring their preferences and needs are central to care planning and decision-making.

Salary: £30,000 - £42,000 per annum (dependent on experience). Access to continuing professional development opportunities and a supportive working environment are also available.
