We are currently recruiting for a Homecare Nurse Specialist to work in the beautiful county of Shropshire. As a Homecare Nurse Specialist, you will play a crucial role in delivering high-quality nursing care to patients in their own homes.

The successful candidate will have experience in community nursing and strong clinical assessment and decision-making skills. You will work closely with other healthcare professionals to ensure coordinated care and support patients to maintain independence and achieve their healthcare goals.

Key responsibilities include:

  • Clinical Care: Conducting holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
  • Community-Based Care: Managing a daily caseload, prioritising tasks and responding to changes in patients' conditions.
  • Team Collaboration: Participating in multidisciplinary team meetings and case conferences to support care planning and review.

Salary: £29,000 - £40,000 per annum (dependent on experience). A full UK driving licence and access to a vehicle for work purposes may be required.
