This is an exciting opportunity for a motivated and compassionate Band 5 Community Nurse to join our team in Shropshire. As a Community Nurse, you will play a vital role in delivering high-quality nursing care to patients in their own homes or community settings.

Your primary responsibility will be to provide holistic nursing care to patients, including assessing their needs, developing personalized care plans, and implementing interventions to support their health and wellbeing.

Responsibilities and Requirements
  1. Assess patients' needs and develop personalized care plans
  2. Deliver high-quality nursing care to patients in their homes or community settings
  3. Work collaboratively with other healthcare professionals to ensure seamless patient care

To be successful in this role, you will need excellent communication skills, a caring attitude, and the ability to work independently and as part of a team.

Benefits

An estimated salary of £32,000 - £42,000 per annum, depending on experience
