**Purpose of the role**:
We are looking to employ a Care Coordinator; you will be responsible for the rotas to provide continuity of care for both our clients and support workers.

Being a Care Coordinator with Sentinel Care Services is about more than just rota management. You will be involved with supporting with Recruitment, Business Growth and ensuring we are always delivering the highest possible standard of care.

Sentinel Care Services are an established and expanding domiciliary care company. We have been operating for over 25 years offering excellent care to our clients within their own home.

Our Care coordinator role is for 24 hour per week. The hours are typically worked between 8am and 4:30pm Monday to Friday additional to this on call is required to be covered weekdays and alternate weekends.

**Key responsibility**

Ensure care workers are appropriately allocated to clients so that care is delivered on time, safely and in line with clients wishes as agreed in their care plan.

**Duties**

Liaise with the recruitment officer to make sure sufficient support workers are recruited to meet the needs of the business.

Arrange cover for support workers sickness and holidays.

Accept new packages, allocate and process new referrals for care promptly.

Process changes to clients care plans.

Make sure there is sufficient cover to deliver care on time.

Monitor the allocation of support workers to maximise efficiency whilst supporting them to maintain an appropriate work/ life balance.

Prioritise when there are unexpected emergencies.

Caring, compassionate, and understanding towards people in need of care and support.

Self-motivated and keen to learn. Willing to seek guidance when needed and follow instructions.

Excellent timekeeper and reliable with a professional, smart appearance.

Ability to communicate clearly and build positive working relationships with clients their families, staff and other health and social care professionals. Good telephone manner, ability to problem solve alone and in a team.

Excellent organisational skills
- proven ability to plan and organise and prioritise workloads effectively so that clients receive the services they expect and are safe.

Ability to maintain clear and accurate records and follow statutory reporting procedures.

Good administrative and IT skills. High level of accuracy and attention to detail.

Ability to think clearly when under pressure. Comfortable using own initiative to solve problems or as part of a team especially in an emergency.

To follow Sentinel Care Services Care’s policies, procedures, and instructions at all times.

Appropriate professional qualification

Previous experience in domiciliary care environment is ideal but not essential.
