Herts Valleys Adult Social Care is in need for 2 highly motivated and experienced HCPC registered locum Occupational Therapists to work with vulnerable older adults with physical disabilities or with dementia and their carers, They will receive support andsupervision from an Occupational Therapy manager or senior practitioner within the area

You will work preventatively by completing strengths based and creative assessments to achieve or maintain well-being and enable older adults to remain in their own homes with the optimum level of independence.
You will be expected to ensure that frailer elderly people are transferred using recognised safe practice and be able to evidence up to date moving & Handling training
You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes.

You will keep the service user at the centre of all activity and draw on the vulnerable persons strengths and assets.
A good understanding of the relevant adult social care legislation including safeguarding risk assessments and positive risk taking are essential requirements.

You will need to be able to work autonomously within agreed timeframes and manage frequently changing circumstances.
You will work as part of an integrated team, attend multi-disciplinary meetings and need to have excellent communication skills.
You will work as part of the wider health and social care services and voluntary services and attend multi-disciplinary, family and safeguarding adults meetings.

You will hold a relevant professional qualification and hold up to date registration for practice with your professional body. You will have experience in working with the older people and their carers. You will possess sound knowledge of NHS / Social CareLegislation and have good understanding of the safeguarding policies governing provision of services to older people demonstrable abilities in your in day to day practice.

The Early Intervention Vehicle Service takes positive risks and works creatively and flexibly to enable older people in the whole of West Hertfordshire to remain in their own home following an acute episode. Professionals have immediate access to assistiveequipment and are expected to work closely with local health, social care and voluntary services to promote

Must be a Car Driver with Business insurance
