**Receptionist**: The Receptionist plays a key role in the service by providing a professional, responsive point of reference for all out-of[1]hours contacts in the Primary Care Centre (PCC). This role performs the vital link between patients and duty GP and the post holder will be responsible for processing the out of hours consultations in line with established guidelines and procedural protocols following call handling.

**Driver**: In addition to the duties of the Receptionist the Driver will convey the OOH Duty GP to the home or place of residence of patient’s whose clinical condition makes it difficult for them to travel to any of the Primary Care Centres. A vehicle suitable for this purpose will be provided by the SBUHB Health Board.

The post holder will be required to work a range of flexible shifts patterns i.e. overnights, evenings, weekends and Bank holidays they will also be required to work across our Morriston and Neath Port Talbot bases.
- Receptionist
- Provide a reception service for patients who attend the GP Out of Hours Service on their arrival at the Treatment Centre.
- Provide a professional response to all Health Professionals who contact the service in person or by telephone
- Manage patients referred from the Emergency Department in accordance with agreed protocols.
- Arrange appointments in the Treatment Centre. This will involve contacting patients by telephone and may require the use of tact and persuasive skills.
- Liaise with the GP undertaking Home Visits to ensure that the visits are undertaken within timescales appropriate to the clinical priority and to make comfort calls to patients / carers when delays occur.
- Develop good working relationships with OOH GPs, staff in the 111 Call Handling Centre and other Health Professionals who contact the service.
- Enter patient details on the system ensuring that the WDS look up is completed
- Deal with general telephone enquiries and to refer those requiring patient care/advice to nursing staff
- Driver

Receive in the GP’s absence demographic data relating to visits to be undertaken and ensure that the GP contacts the PCC to obtain clinical information as required.
- Plan journeys in discussion with the GP to ensure that visits are undertaken within the timescale appropriate to the clinical

priority of the patient.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

- Arrange appointments in the Treatment Centre in accordance with timescales appropriate to the clinical priority. This will involve contacting patients by telephone and may require the use of tact and persuasive skills to encourage attendance at a Treatment Centre.
- Receive direct referrals from Health Professionals in accordance with agreed protocols. This will include Paramedics, Community Nursing Staff, ED Clinicians.
- Liaise with the GP undertaking Home Visits to ensure that the visits are undertaken within timescales appropriate to the clinical priority and to make comfort calls to patients / carers when delays occur.
