We are looking to recruit a Multidisciplinary (MDT) Coordinator to provide vital administrative support to our clinical teams in our West Kent area.

This is predominantly an administrative role. Our administration team is the backbone of our service, providing the support needed to enable the clinical staff to offer an excellent service to our patients.

In this role you will be responsible for effectively managing and chairing the patient Multidisciplinary meetings. This work brings together a core group of Health & Social Care colleagues to discuss patient care, and plan treatment options and care outcomes.

The MDT meetings support integrated working between Primary care and community based services, allowing the opportunity for a core health and social expert group to discuss specific patient care delivery. This proactive care supports decision making and further health & Social care delivery to ensure the patient is well supported within the community setting.

You will be preparing the meetings and ensuring all information is cascaded out to participants beforehand. Following the meetings you will be recording and following up on the actions agreed and ensuring patient records are updated accordingly.

You will need to have very good communication skills, be skilled and confident in chairing the meeting, and keeping to time. You will have very strong IT skills and must be confident in picking systems quickly.

**Additional Working Pattern**: Monday to Friday 8-4
