Clinic Coordinator

As a clinic care coordinator, you will be responsible for organizing and coordinating homecare services for our clients. Your role is essential in ensuring that our clients receive the highest standard of care, tailored to their unique needs. You will also provide support to our dedicated team of care workers.
**Responsibilities**:
**1.** **Service Users**:
- Assess and process referrals for new homecare packages.
- Handle changes and amendments to existing homecare packages.
- Match and assign care workers to service users while adhering to organizational standards.
- Collaborate with administrative team members to ensure top-quality service delivery.
- Maintain open communication with all relevant parties involved in providing care to service users.
- Create, manage, and adjust complex staff schedules, ensuring adequate coverage and timely updates on the rostering system.
- Provide on-call service outside regular working hours and maintain detailed on-call activity logs.
- Coordinate and implement service standards as per company policies, maintaining detailed records and documentation.
- Supervise and support the carer team, including line management and care instructions to service users.
**2.** **Staff Supervision and Development**:
- Organize and deploy staff efficiently within a locality, consulting with the Manager as needed.
- Ensure staff workload aligns with their skills, experience, and responsibilities.
- Conduct staff supervision and Performance Review and Development meetings in line with service objectives.
- Ensure staff compliance with statutory requirements, company policies, and procedures.
- Identify team training and development needs and oversee training provision.
- Initiate absence management procedures, including return to work interviews.
- Assist with induction training as directed by the Manager.
- Conduct NVQ assessments and attend relevant training and development programs.
- Arrange and conduct team meetings as required to facilitate effective communication and updates on procedures and policies.
- Supervise the operational team in the Manager's absence or direction.
**3.** **Quality Monitoring**:
- Assist in quality monitoring processes as directed by the Manager, including site visits and health and safety records/risk assessments.
- Address concerns arising from quality visits or issues with private sector providers as requested by the Manager.
- Monitor service quality and contribute to Quality Assurance and performance measures.
- Conduct Service User reviews and maintain a complaints and compliments file.
**4.** **Liaison**:
- Collaborate with relatives, carers, volunteers, healthcare professionals, and other internal/external departments to ensure appropriate service delivery.
5. **Miscellaneous**:
- Undertake any other duties reasonably required by your supervisor to meet service needs.
- Uphold the organization's Equal Opportunities Policy and policies promoting harassment-free and discrimination-free environments.
- Ensure the health and safety of all individuals and resources while at work, operating within safe working practices.
- Cooperate with the organization's health and safety policies.
- Promote diversity and counteract discriminatory practices.
