**About the role**

The Dementia Adviser will be the first point of contact for support, information and onward referral as needed throughout the person’s life with dementia or until residing in residential nursing care.

Dementia Advisers (DA) will be based in the community to provide a single, identifiable point of contact for persons diagnosed with dementia and their carers. They will work closely with health and social care services and develop local links and networks to support persons with dementia, including under
- served communities.

The DA will work in partnership with the Community Dementia Nurse and GP to:

- Share knowledge of local resources and services for dementia
- be a named contact for the person diagnosed with dementia and their carer.
- Develop skills to respond to a range of issues and problems that might arise, referring to appropriate specialist services.
- Provide at least annual contact with the person with dementia living in the community from diagnosis to end of life or moved to residential care.
- Be an active member of the multi-disciplinary team contributing to the planning and delivery of individual needs, care and support.
- Support peer support networks with local partners.
- Work closely with community dementia nurses and GPs to ensure holistic review of care and treatment.
- Manage and prioritise own caseload, in accordance with the health and wellbeing needs of their population and where required and as appropriate, refer people with dementia back to other health professionals.

- A highly motivated individual, with an understanding of dementia and the needs of people affected by dementia.
- Be empathetic and non-judgmental in your approach with a commitment to equal opportunity.
- Have good communication skills to meet the diverse needs of our community and represent their needs to statutory and other voluntary agencies.
- Understand client confidentiality and how this is applied when representing client needs.
- Preferably had some experience of working with a wide variety of relevant agencies.
- You will possess demonstrable experience in the healthcare industry.
- Have the ability and means to be able travel independently
