We are looking for kind hearted and dedicated home care Coordinator/ carer to perform the following tasks:

- To produce weekly rotas, including;
- Liaising with carers regarding their availability;
- Liaising with clients about their requested calls;
- Liaising with care managers regarding changes to packages of care;
- Sending weekly schedules of care for clients;
- Daily trouble-shooting, including;
- Covering staff illness or absence;
- Allocating extra calls in the event of client illness;
- Cancelling calls when appropriate.
- Managing care scheduling software, including;
- Adding staff and updating details;
- Updating training matrices when required;
- Updating service requirements.
- Ensuring enough capacity to provide continuity of care, by;
- Managing staff holiday requests;
- Liaising with HR administrator regarding roles required;
- Making decisions regarding taking on new packages of care.
- Communicating with clients and carers, including;
- Support via telephone;
- Sending letters to clients;
- Arranging meetings and interviews when required.
- To monitor the performance of your team members, including;
- Regular, unannounced spot-checks at the client’s home;
- Regular, planned supervision sessions;
- Feedback collection from clients and other team members;
- Auditing of daily notes and MAR Charts.
- To keep morale high within your team, by;
- Identifying problems and issues early and reporting to Care Manager;
- Organising events and meetings for your team;
- Aiding communication between your team and the office.
- To help the training and induction of new starters, by;
- Arranging shadowing and supervision calls.
- To help the Company grow and take on extra clients, including;
- Providing excellent, responsive service to existing clients;
- Working with the Care Manager and Referrer to fit in extra care packages;
- To prioritise larger and longer lasting care packages when making capacity decisions;
- To develop strong relationships with referrers.
- To cover calls when new packages of care start which cannot be immediately covered by existing carers.
- To work as part of the Good Oaks team, which includes;
- Take part in staff and customer meetings;
- Training activities as directed;
- To read and write reports;
- To attend appraisal and development meetings;
- To communicate in various formats.
- To allow Good Oaks to provide continuous, reliable service by;
- Taking part in the on-call rota when required;
- Undertaking care calls when required, including outside of office hours, such as mornings, evenings and weekends;
- Assisting the on-call when requested.
- To comply with the Good Oaks business guidelines and policies at all times.**Expectations and Qualifications of a home care assistant**:

- We expect our home care assistants to have a compassionate and caring approach to their duties;
- Reliable and flexible;
- Willing to learn and progress with the company;
- Experience in domiciliary and residence (desirable but not essential)
- Home care qualifications are desirable but not essential as professional and paid training will be provided.

The Care Coordinator/ carer reports to: Care Manager / Director

**Salary**: £18,000.00-£21,000.00 per year
