**Job Description**:
The main purpose of the** Care Coordinator** is to be responsible for all tasks related to helping with new client starts including matching and scheduling care professionals to clients. This role would suit an individual who enjoys solving puzzles and dealing with problem solving and implementing solutions.

As the **Care Co-Ordinator**, you will;
- Coordinate the forward planning of care schedules and ensure it is consistently achieved in order to provide continuity of care to our clients
- Ensure that any communication with external sources such as next of kin, GP’s, nurses, physiotherapists and any other professionals involved is prioritised if they impact on the care and wellbeing of the Client
- Ensure that all Care Professional holiday, sickness, unplanned absences and all emergency calls are documented electronically;
- Covering care calls and gaps in the care schedules where they arise and cannot be covered
- Be part of the operations team on call rota system during evenings and weekends as and when required
- Understand the arrangements for ensuring all clients are safeguarded against the risk of abuse and respond appropriately to any allegations of abuse, or complaints regarding service delivery whilst complying with Home Instead policies and procedures.

**Qualifications**:

- Need to have a high level of attention to detail gained in a previous customer service orientated role;
- Require a strong knowledge of IT systems
- Be** **self-motivated and demonstrate integrity, trustworthiness, reliability;
- Ability to build and maintained strong working relations with clients and colleagues alike;
- Previous experience working within social care is preferable, but not essential;
- Experience of co-ordinating rotas or schedules is also desirable, but not essential.
- Hold a valid driving licence and have use of your own vehicle.
