Oxford, United Kingdom Oxford Health NHS Foundation Trust Full time

Would you like to work in an innovative CAMHS Tier 4 service?

Are you interested in delivering intensive community support to young people and their families, as an alternative to hospital admission?

Are you keen to work with highly motivated and passionate colleagues?

LDA Hospital at Home is an innovative new service being developed under the Thames Valley Provider Collaborative. The Hospital at Home team will focus on young people with moderate to severe learning disabilities AND/OR a severe degree of functional impairment associated with an autism diagnosis who are at risk of home or placement breakdown and who could meet the threshold for inpatient care.

In some cases, the team will help prevent unnecessary admissions and in others, young people will be stepped down from tier 4 care through the LDA Hospital at Home team, shortening their inpatient admission. LDA Hospital at Home maximizes the use of new technology, provides care close to home, reduces admission, length of stay, waiting times and provides earlier discharge from inpatient care.
By managing the care pathway, we will be able to ensure that children and young people are cared for in an inpatient setting or via an ‘out of hospital’ service as close to home as possible. This will:

- Reduce the burden on families to travel long distances
- Enhance continued engagement with community clinical teams
- Encourage early therapeutic home leave
- Minimise Length of Stay
- Avoid admission

The team will have a phased implementation;
- During Phase 1,you will be part of the Senior MDT providing consultation and support across the footprint of the Thames valley collaborative. This may involve some planned assessments and support.You will alsobe involved in the training and preparation of the team’s junior staff members in anticipation of phase 2
- During Phase 2, in addition you will be responsible for providing support/ supervision to a number of staff and contributing to the ad hoc remote supervision of clinical material that relates to psychological interventions for people with either a learning disability or autism. This may include interventions that manage behaviour that challenges
- During Phase 3,

you will continue to support the team during its expansion in line with your clinical competencies as a psychologist and work towards developing the skills of individual team members directly through supervision and in-directly through contributing to team process and understanding

Oxford Health NHS Foundation Trust provides physical, mental health and social care for people of all ages across Oxfordshire, Buckinghamshire, Swindon, Wiltshire, Bath and North East Somerset.
