LDA Hospital at Home is a specialist service working with patients up to 18 years old with moderate to severe learning disability and autism. The Thames Valley Provider Collaborative is a network of service providers covering Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and Bath and North East Somerset (BaNES), working collaboratively to provide high quality care for the most unwell young people as close to home as possible. We are currently recruiting our multi-disciplinary team (MDT) who will work with young people diagnosed with moderate to severe learning disabilities and autism, who are at risk of admission due to mental health difficulties. You will be based in Oxford in our new offices located above the meadow unit on the Warneford site.

To undertake, coordinate and monitor, as an autonomous practitioner, the comprehensive functional assessment and delivery of person-centred healthcare and produce detailed behavioural assessment reports. To conduct assessments and observations, construct behaviour support plans and implement and evaluate these plans for service users. Utilise clinical skills and advanced clinical reasoning to recommend the best course of intervention, develop comprehensive Care Plans, oversee their implementation and evaluation and set the standards of care, assessment of care needs and their development, implementation and evaluation of programmes of care within the locality using evidence-based practice. To work in partnership with senior colleagues and others to develop educational programmes in relation to Applied Behaviour Analysis and Positive Behaviour Support, for registered and unregistered health care professionals within the Intensive Support Team.

NB: Access to a car or alternative travel for business use is essential. This is a hybrid, office and home-basedrole. To carry responsibility for a defined caseload across a range of settings including, but not limited to, staffed settings, family homes, schools, and residential services and/or offer specialist clinical advice as a member of a multi-disciplinary team. To be professionally, legally responsible, and accountable for all aspects of your own work including the management of patients in your care.

To present and communicate complex clinical information in such a manner as to motivate and inform clients, carers, families, and members of the multidisciplinary team /other professions.

To collect, collate and analyse data/ information regarding the effectiveness of behaviour support plans and to produce reports using Microsoft Office effectively for the purpose of audit, research and service performance. Identify new ideas, innovations, methods and developments to encourage the uptake of education and training that demonstrate positive outcomes and improvements in patient care, enable role development and meet the needs of the service. To work in partnership with senior colleagues and others to develop educational programmes in relation to Applied Behaviour Analysis and Positive Behaviour Support, for registered and unregistered health care professionals within the Intensive Support Team. To receive clinical supervision, and to undergo and actively participate in own performance appraisal, identifying ones own development needs and set personal development objectives in discussion with appraiser.
