Position: Qualified Adults Social Worker
Location: Warrington Borough Council
Pay Rate: £30 per hour

Key Responsibilities:
Navigate within the legal frameworks including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983
Provide invaluable support to individuals with special educational needs and disabilities as they transition into adulthood
Collaborate closely with multidisciplinary teams to tailor personalized care plans.
Guide and assist individuals until they reach the age of 25, ensuring a seamless transition into adult services

**Requirements**:
Qualified Social Worker with demonstrable experience in adult services
Thorough understanding of legislation governing adult social care
Possess empathy, compassion, and a genuine commitment to improving the lives of vulnerable individuals
