The post-holder will be clinically experienced in the field of older people. S/he will practice at an advanced clinical level and exercise high levels of judgement, discretion and decision making in clinical care. S/he will ensure person-centred care and family carer collaboration. As appropriate, the promotion and maintenance of independence, health and wellbeing will be undertaken.

The post will require you to appropriately assess, examine, investigate, diagnose and treat patients with undifferentiated and undiagnosed acute presentations, focusing specifically on frail elderly people who have complex needs. Admission avoidance and early, supported discharge are key aspects of this role and involve risk assessing those people whose care does not need to be delivered in hospital.

This post aims to support new ways of working across acute and community services. We are looking for innovative, forward thinking clinicians who have a background in Advanced Practice, who are independent non-medical prescribers and hold a Masters in Advanced Practice. The role is dynamic and the post-holder will be proactive in developing this further in response to challenges within frailty and advanced practice. Successful applicants will be supported in their development by a Consultant mentor, a robust appraisal process and allocated time for continuing professional development

- The main duties and responsibilities relate to the independent assessment and management of patients who present to acute medicine. A scope of practice will be agreed and reviewed annually as the knowledge, skills and competence of the Advanced Practitioner develops.
- To appropriately assess, examine, investigate, diagnose and treat patients with undifferentiated and undiagnosed acute medical presentations
- To carry out comprehensive assessments of patients as an autonomous practitioner particularly those with highly complex presentations
- To order interpret and act upon medical investigations and expedite access to appropriate medical staff
- Independently adjust treatment plans (pharmacological and non-pharmacological) according to need assessing appropriate management options and initiate treatment or referral to an appropriate professional where necessary
- To demonstrate advanced critical thinking and analytical skills to support clinical judgement and decision making process
- Communicate effectively with a wide range of people in a manner consistent with their level of understanding, culture and background
- Working in a collaborative manner with all other members of the multidisciplinary team, fostering an environment that is supportive of professional respect and equality.
- Contributing to the identification of training and development needs within the business group delivering multi-professional education and training as required.
- Strive to improve clinical quality and maintain professional standards within the Integrated Care Business Group
- Work with Consultant (Nursing and Medical) colleagues to develop evidenced-based clinical pathways, protocols and guidelines based on national guidelines with a view to continuously improving quality and managing clinical risk more effectively.
- To obtain regular clinical supervision and feedback from Consultant mentor, maintain a portfolio of evidence that reflects the maintenance and on-going development of clinical skills.
- Regularly audit own clinical practice identifying areas for training and development.
- Identify own development needs, take responsibility for own personal development and maintain a PDP, presented at annual appraisal.
