The community Dietitian Team at Stockport NHS Foundation Trust have an exciting opportunity for a specialist Dietitian with experience in frailty to join the service. This is a maternity cover fixed term post. The role will provide dietetic support and service development for this vulnerable group of the population, as well as covering discharge to assess bed bases. The successful applicant will also lead on Dietetics and frailty agenda.

We are looking for an enthusiastic and motivated Dietitian to join our Integrated Nutrition and Dietetic service at Stockport NHS Foundation Trust. The role will be part of the growing community frailty service at the Trust.

Work as an autonomous specialist dietitian to provide high quality service to agreed standards

Provide specialist Dietetic treatment to delegated caseload of Frailty patients using evidence based patient centred principles to assess, implement and evaluate interventions

Regularly evaluate treatment plans to ensure they are safe and effective

To develop the Dietetic frailty service and act as a clinical specialist alongside the MDT

To provide advice on the use of oral nutritional supplements, enteral and parenteral nutrition as required

Act as an independent practitioner and be professionally accountable and responsible for aspects of own work. Decide priorities for own work, balanced patient related and professional demands in consultation with Team Leaders

To work alongside consultants, specialist nurses and ward staff to provide specialist nutritional treatment plans as part of holistic patient care

Provision of expert advice on the management of patients referred to the nutrition team due to additional complex clinical and nutritional needs and liaise with Team Leader and Specialist Dietitians within the department to agree appropriate management

To attend ward rounds, MDT meetings and best interest meetings as needed, promoting an understanding of the role of diet and nutrition in achieving the best outcome for each patient

Provide training to healthcare professionals regarding nutritional care and interventions, including presentations

To lead and develop the dietetic intervention of the Frailty pathway and MDT working

Be responsible for planning, coordinating, delivering and evaluating clinical case work within the Frailty patient group

Be responsible for planning, coordinating, delivering and evaluating clinical case work within the frailty service

Provide a key role in partnership working with patients, carers, the MDT, and wider statutory, private and voluntary sector services.

To take responsibility for staff and student training and development.

To provide evidence-based care, education and information to patients, carers and health care professionals. This will be in a range of inpatient, outpatient settings and community home visits.

To take a lead with quality improvements in the Frailty Service

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
