Associate Community Matron

Job Introduction
Are you a thoughtful, dedicated, dynamic nurse who is passionate about holistic patient care? An exciting opportunity has arisen, to appoint an Associate Community Matron based in Midsomer Norton.
As an Associate Community Matron you would work closely with the Matron, Primary care providers and other members of the Multidisciplinary team to support patients in the Adult Community setting who are vulnerable and have complex healthcare needs. You will case manage your own group of patients, with the support of the Matron, providing clinical assessments and intervention for adults in the community.
You will be based primarily in Midsomer Norton, but the Community Matron team works across BaNES, covering the areas of Midsomer Norton, Radstock and Bath areas. **Because of this you must have a UK license and have access to a car for work purposes**. You will participate in the duty matron rota to cover the service.

Main Responsibilities
Within this role, you will:
- Be responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
- Implement, monitor and maintain high standards of care delivery at all times.
- Care for patients and their families in the way that you believe is best.
- Deliver our high service standards, sharing best practice, and actively improving the way we work.
- You will be an NMC Registered Adult Nurse and have studied at degree level.
- A qualification in Physical Assessment and Clinical Reasoning skills would be advantageous.
- Having the V300 - Independent Nurse Prescriber Qualification would be beneficial. You would be encouraged to gain this qualification if you have not already achieved it.
- You will have a full UK driving license with access to a car for work purposes.
- You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
